The Junior Clinical Data Abstractor will work under the direction of the Lead Data Abstractor to review patient files and abstract (extract) key data elements from our client’s electronic medical record and enter them into our proprietary database to help with ensuring compliance with nationally recognized quality patient care processes and clinical outcomes. The ACC registries include Chest Pain-MI Registry, Afib Ablation Registry, CathPCI Registry, PVI Registry, STS/ACC TVT Registry, ICD Registries, IMPACT Registry, and LAAO Registry.
Reviews patient records and abstracts key data elements from electronic medical records according to registry guidelines
Submits accurate validated data into abstraction software using various registry software applications
Provide data analysis to reporting hospital managers, as appropriate
Maintain security and confidentiality of medical records and Protected Health Information (PHI)
Consistently meets or exceeds productivity and accuracy standards established by the organization
Uses good judgement to prioritize daily operations and complete assigned tasks to meet reporting deadline and /or internal requests.
Other duties as assigned by employee’s supervisor
Supervision is provided by the Lead Data Abstractor, Registry site manager, and Director of Carta Healthcare. Other mechanisms of supervision used may be:
Direct on-site, electronic or phone supervision by Lead Data Abstractor
Chart screens on random charts by the Lead Data Abstractor or designee.
Chart audits on random charts as an integral part of selected quality improvement programs.
Ongoing Professional Practice Evaluation (OPPE) through tracking of performance indicators.
The Junior Data Abstractor will receive a formal annual performance evaluation by the Lead Abstractor, Registry Site Manager, and Directorship from Carta Healthcare.
Any combination of education and experience that would likely provide the required knowledge, skills and abilities as well as possession of any required licenses or certifications is qualifying.
Basic medical terminology
Electronic health record experience (preferably EPIC)
Knowledge of computer hardware and software (proficiency with the Microsoft Office and Google Suite apps, a plus).
Detail oriented, dependable and possess excellent communication skills
Exposure to the practice of Cardiology and CV registries is a plus
Flexible and willing to work a varied schedule to meet the needs of the organization and must be available to team members and for scheduled meetings.
